We accept many different insurance plans. Each individual insurance plan may vary in coverage and with the ever changing health insurance market we strongly encourage you to consult with your specific plan to determine what your coverage will be. Questions to ask include:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?

*We are out of network with Anthem BCBS. The self pay rate will apply, however we will still bill your insurance if it is primary. Check with your insurance company to see if you have Out of Network benefits. All therapy sessions will be 60 minutes.
*Due to being out of network with Anthem, we cannot accept Anthem primary with Ohio Medicaid secondary. **We are not able to accept self pay**
We will work with you to provide an estimate you will owe for each appointment. Please contact your insurance company to provide you with more detailed information regarding your benefits.
Self Pay Rates:
Initial diagnositc assessment: $150.00
60 min therapy session: $125.00

Payment
Cash (exact amount only), Credit, Debit and HSA/FSA cards are accepted. We require a card on file at the time of scheduling. Payments are due at the time of your appointment.
Cancellation Policy
Late Cancellation/Missed Appointment Fee (Less than 24-hour notice): $80.00
*Please call our office at 330-433-2688 (option #2) to cancel an appointment. If you leave a message, your appointment cancellation will be documented at the time your message is received.
